Date: March 8, 2000SUBJECT: Misprinted Stock of Standard Form 86, Questionnaire for National Security Positions
It has just come to our attention that most copies of the Standard Form 86 (SF 86), Questionnaire for National Security Positions, distributed by the General Services Administration (GSA) during the last half of 1999, have numerous typographical errors, and must be destroyed and replaced. GSA estimates that there are 322,800 bad forms in the hands of customers. GSA is in the process of contacting all customers who ordered SF 86s to request immediate disposal of the bad stock and to arrange replacement at no cost to the customer. The bad forms were shipped from stocking depots in Palmetto, Georgia; Burlington, New Jersey; and Stockton, California.
At this time, GSA has pulled all bad stock from their inventory, and has a supply of 37,600 good forms to meet demand. GSA estimates a one-week shortage of SF 86s may occur while replacement stock is printed.
The bad forms are easy to identify:
For new packages, incorrect forms can be identified from a label inside the plastic wrapping. A "jacket label number" is located in the bottom right hand corner. The incorrect forms have jacket label numbers 760-044 or 560-043.
For individual forms, there is an incorrect NSN number at the top right corner of the form. The incorrect forms have NSN 7540-00-634-436. The correct NSN is 7540-00-634-4036. Another quick indicator is that 7 is printed twice as a question on Page 1 of the form.

In addition to the many grammatical and typographical errors in these forms, there is one material error that requires corrective action by submitting offices.
OPM's Federal Investigations Processing Center will process investigations initiated with the incorrect forms until April 1, 2000, as long as they are accompanied by the correct release.
- The Authorization for Release of Information (page 10) is incomplete, and must be replaced with the page 10 Authorization from a valid SF 86. A photocopied replacement of page 10, with an original signature of the subject, is satisfactory.
